• The Membership application must be completed in full - incomplete forms will not be accepted.​

​

• AMA membership will be verified through AMA.  If AMA is not active, application will be rejected.

​

• The Membership initiation fee of $100.00 and Club dues of $100.00* is to be paid at the time the application of Membership is submitted.

 

*If applicant joins between January and June the membership fee is $100.00.  If applicant joins between July and December, the fee is $75.00.
